• Photos must be 2 inches x 2 inches in size, in color.
• The physical photo must be affixed to the physical application form.
• And the digital photo must be uploaded on the OCI form online.
• The Physical and uploaded photographs must be identical.

• The digital signature must also be uploaded online on the OCI portal (only in blue or black ink).
• The uploaded signature must match the signature on the physical application form and passport.

• With a validity of a minimum of 6 months at the time of submission of the application.
• The passport's information page should be duly signed at the designated space.
• The document must be self-attested.

It must reflect.

• Names of parents;
• Date of Birth; and
• Nationality.
• Note: Birth certificates issued by Hospitals are not accepted.
• If the applicant was born outside of India or USA, a Birth certificate copy must be apostilled or attested by the foreign mission concerned is required.

• Parents or Grandparents' first 2 and last 2 pages of the Indian passport copies. (Mandatory Requirement)
• Copy of school leaving certificate in India, OR
• Copy of Indian Voter ID Card/PAN Card/Indian Driving license (even if expired is ok); OR
• Copy of the Domicile Certificate issued by  the  Competent Authority; OR
• Copy of Nativity Certificate from the competent authority, OR
• OCI Card/ PIO card of parents/ spouse along with the base papers/ documents upon which the OCI/ PIO card was issued.

• If one or both parents hold a US nationality, please submit a copy of the US passport information page.

Proof of address must match with the present address as per the application. Copy of any one of the following documents may be provided as proof of address:

• State-issued Identity Card, OR
• Driving license, OR
• Utility bill – water/gas/electricity/telephone (Latest one, not more than 3 months old), OR
• Valid Lease Agreement, OR
• Mortgage, OR
• Income Tax Return, OR
• For college/university students living in university housing/dormitories, a letter from the institution on official letterhead giving details of the applicant’s present address, OR
• In case of the applicant residing with parents/children/relatives, a notarized undertaking from them with any copy of address proof as indicated above.

• Copy of the marriage certificate, if married.
• Copy of the divorce decree, if divorced. If re-married, include a copy of the marriage certificate.
• Copy of the Death certificate, if the spouse is deceased.
• If proof of marital status is not available, a notarized affidavit may be submitted, thereby confirming the marital status, and indicating the reasons for the non-availability of the requisite certificate.

• The applicant must write and sign a consent letter in their own words confirming the Indian consulate / Embassy is authorized to make an edit and necessary corrections on observing errors in the OCI application

• Application confirmation letter/payment confirmation is mandatory. Complete the process online on VFS India Consular Application Centre’s website to get the application confirmation letter/payment confirmation.
